- Perform complete and detailed nutritional assessment of each patient based on health history, medical conditions and energy requirements for comprehensive management of their health problem
- Determine client-specific nutrient & energy requirements with consideration to specific lifestyle, addressing individualised dietary requirements.
- Plan nutritious meal options for individuals with various health needs in health facility & facilitate comprehensive training of care takers/clients on nutritional needs of individuals in their health management
- Assist clients with meal plan development and address their individualised dietary requirements
- Coach individuals in methods of managing chronic diseases with dietary plans (nutrition education and counselling)
- Health educate expectant mothers, nursing mothers and women of child bearing age about their nutritional needs and benefits of breastfeeding infants to the recommended age
- Implementation of supplementary feeding of the moderately malnourished under five, pregnant and lactating mothers
- Create awareness on the importance of good nutrition practices on children and family health in general
- Conduct massive screening for malnutrition for children under five regularly in collaboration with community health volunteers
- Train community health workers and target community on the purposes of good nutrition and health practices
- Support systematic delivery of health and nutrition education messages to the community and facility levels
- Work with local health authorities to ensure regular immunisation of children
- Follow up nutrition programs and prepare relevant reports for stakeholders in the program
- Ensure that food supplements are stored in a safe and secure manner free from contamination and theft
- Manage quality assurance programs including on site evaluations, internal audits & customer surveys
- Carry out home visits for discharged patients to ensure adherence to nutrition program set
- Any other duty that may be assigned by accredited supervisors
